Additional Resources Advance Directive An advance directive is a legal document that you complete to identify your medical decision makers (we recommend at least 2 agents) and your treatment preferences in the event that you could not communicate them for yourself. Here is an example of an advance directive for health care. When you are completing this form, it is important that you have two people witness your signature at the end. These cannot be the same two people that you name as your medical decision makers. You can ask friends or neighbors to watch you sign the form. You can also take it with you to the customer service desk at the grocery store or bank. It does not have to be notarized but they can sign as a witness. Will A will is a legal document that allows you to designate how you want your estate and belongings distributed after your death. This is separate and un-related from your health care. Medicare is a federal health insurance program that covers some of the cost of health care for people in the United States. Generally, Medicare is for people who are 65 or older, but some people under 65 may be eligible if they have a disability, End-Stage Renal Disease (ESRD), or ALS (Lou Gehrig's disease). Medicare is run by the Centers for Medicare & Medicaid Services (CMS) and has set standards for coverage and costs. Medicare beneficiaries pay part of the costs through premiums, deductibles, and coinsurance. Medicare has four parts: Part A (Hospital Insurance) Part B (Supplementary Medical Insurance) Part C (Medicare Advantage) Part D (Medicare Prescription Drug Benefit). Medicare covers many preventive services, like shots and screenings, and some doctors' services, outpatient care, medical supplies, and more
Generally, you only need to sign up for Part A and Part B once. Each year, you can choose which way you get your health coverage (and add or switch drug coverage). Medicare is different from private insurance — it doesn’t offer plans for couples or families. You don’t have to make the same choice as your spouse.You’ll have Original Medicare unless you join a Medicare Advantage Plan.
Senior Advantage Plan Benefits - Transportation Most counties offer transportation at a minimal cost but they do not cross county lines. So if you live in Cobb county and your doctor's appointment is in DeKalb county, they can get you to a transfer point but they can’t get you all the way to your doctor's appointment. Empowerline covers several metro counties. If your patient lives further out, you can advise them to call the ADRC (Aging and Disability Resource Connection) @ 1-866-552-4464 or visit their website: https://www.georgiaadrc.com/how_do_i/contact_us/index.php If your patient requires wheel chair assistance or is stretcher bound, the resource counselors at Empowerline or ADRC can provide a list of non-emergency transportation companies but this is an out of pocket cost that most seniors (living on social security and/or a small pension) cannot afford. It’s usually $250 per leg plus mileage.

Marta Mobility Shared ride, advance reservation mode of public transit. These buses operate in an ADA-designated service area within Fulton, DeKalb and Clayton Counties and the City of Atlanta along a 3/4-mile corridor located on each side of all fixed bus routes and in a 3/4-mile radius of each station. To become eligible to ride MARTA Mobility, you must complete an application and an in-person interview and assessment. Download the application (click on the link below), or contact the Mobility Eligibility office at 404-848-5389 to request an application for Mobility Services. The application has two (2) parts (A & B) and is the first phase of the process. You must complete Part A and the Part B must be completed by a licensed rehabilitation or healthcare provider who is familiar with your diagnosis. The completed application (both Part A & B) must be faxed to 404-848-6900, or mailed to: Mobility Service, 2424 Piedmont Road NE, Atlanta, GA 30324. Once MARTA has received your completed application, you will be contacted and scheduled for the second phase of the process, which is the in-person interview and assessment. |
